At-Home Care Services
Nothing feels as comfortable as home. Yet, as a person ages, it becomes more challenging to live at home without some level of support. AgeWell Atlanta works with Jewish Homelife and Aviv Older Adult Services to meet the short- and long-term needs of older adults. From occasional companionship, cooking help and assistance with bathing or other activities of daily living (ADLs), to 24/7 care after a hospital stay or for chronic illness management, or even just being a good listener, our expert caregivers and can create a Home Care Plan tailored to each individual. As a loved one declines, AgeWell can help educate and connect families to in-home palliative care and hospice services.
We guide all at-home care services based on the needs of the individual, along with family preferences and doctors’ suggestions. Nurses (RNs and LPNs), certified nurse assistants (CNAs) and companions provide as much or as little support as needed. Care Managers can help coordinate medical, physical and social-emotional care so caregivers, medical professionals and families work in concert with each other. The end goal is to provide a the highest quality of life possible for as long as possible in their own surroundings, whether that is their own home or apartment, assisted living community or nursing home.

At Home Care
Private caregivers selected based on individual needs from a carefully screened, certified staff, from a few hours a week to 24/7 medical oversight or post-surgical support.
In-Home Hospice
Medicare-covered medical professionals provide pain and symptom management, comfort and support when facing end-of-life conditions and decisions, as well as bereavement support for families.
Home Based Palliative Care
Medicare-covered medical service that optimizes quality of life by anticipating, preventing and treating suffering during curative treatment such as chemotherapy, or for chronic conditions such as heart or pulmonary disease.
Home Assessments and Care Plans
Personalized home assessments to maximize a loved one’s independence and quality of life, including recommendations for assistive devices, safety supports, and comprehensive plans of assisted care that can relieve uncertainty, explain available services and overcome confusion about different programs or solutions.
Family Counseling
Education of everyone concerned about appropriate levels of care and resources. Mediation of family meetings to provide a safe space and a neutral party to listen to family concerns and work toward resolutions.
