Senior Care Coordination
Making sure that the right medications are taken at the right times. Coordinating medical services and doctors appointments. Lining up elder care. These tasks can pile up and become overwhelming for families. Aviv Older Adult Services provides AgeWell Atlanta’s seniors the help they need. Through information and referrals, financial assistance, geriatric care, support groups, and hands-on help to access community resources elderly individuals can live a more fulfilling life. All of which is free of charge for qualified participants.
Through Aviv Older Adult Services, AgeWell Atlanta matches case managers with each family. They then assist family caregivers navigate their current situations with care plans that promote self-care and senior at-home care assistance.
Caregivers enrolled in the program may also qualify to receive financial assistance and food from our Kosher food pantry of shelf stable foods, fresh fruits and vegetables. It’s a way to give each senior in-home care help and their families peace of mind.

Individualized Care Plans and Support
Our Geriatric Care Managers can create a care plan to help deal with current and future problems.
At-Home Counseling
From a safety assessment to finding at-home care resources, AgeWell Atlanta can help seniors continue to live independently at home.
Caregiver Support Groups
Counselors support caregivers when they feel the stresses of caring for others as well as living their own life. Discover respite care, support, and resources for elder caregivers.
Access to Kosher Food Pantry
When in need, food insecure seniors have access to JF&CS’ Kosher Food Pantry.
Financial Assistance
When necessary, AgeWell Atlanta and JF&CS is able to help seniors overcome financial obstacles. In addition, Aviv Older Adult Services provides for:
- Holocaust Survivor Support Services
- Older Adult Refugee Support Services
- Older adults and family caregiver meeting income eligibility guidelines
